Most private insurance plans will require us to collect your co-pay at the time of service. We can help you figure this out when you call to schedule your appointment.
We have on-line access to SimonMed, EVDI and Valley Radiology (now Banner Imaging Centers) so you don’t have to bring any images or reports from those facilities. If your imaging was done at other centers it would be helpful to bring any copies you might have at home.
No. We accept most insurances and our payment rates are based upon their determination of reasonable value for the services we perform on your behalf. Because we know that healthcare can be expensive, we try hard to make our patients aware of their out-of-pocket financial responsibility prior to performing the service(s).
During your first visit you will meet with one of our Providers who will perform a comprehensive review of your medical history and perform a physical examination. We will also review any pertinent imaging studies and outside medical records. The Provider will work with you to tailor an individualized treatment plan for your condition and oftentimes order additional testing and therapies.
Most PPO-type insurance plans and Medicare generally do not require referrals. The majority of AHCCS plans do require a referral and our team can help you obtain one from your primary care provider.
